In general, we accept most health insurance plans even if we are out-of-network. If you have out-of-network benefits, we will accept your plan. If you have an HMO or EPO with no out-of-network benefits, we will request an in-network waivers (also called a Gap Exception). This is done through the health plans provider prior authorization process.
